Option to add subject to contact form

Some customer ticket-systems need to use a subject line. Inventing a subject line can be difficult so having a field in Boei populated with it works well. This should be optional.

Some customer ticket-systems need to use a subject line. Inventing a subject line can be difficult so having a field in Boei populated with it works well.
It is now possible to optionally add a subject field in Boei's contact form widget.
Results are shared over Slack, Webhooks, and email.
